A song or a piece? I'm assuming you mean a song.
Start on one side of the equation and build the other from it. Start with a melody and, once you've formed the melody, work on the accompaniment. Or vice versa, come up with an interesting accompanimental figure and figure out a melody that suits it.
Study piano ballads you like.
Choose key you like. For more moving pieces I like to start song in minor and change to equiv major for chorus or bridge. Pretty common tactic and works great. Just stick to common music structures at first. Verse-Chorus-Verse-Chorus-Bridge-Verse-Chorus-Chorus for example. Just work in blocks. Shit isn't quick to do sometimes, so just don't stress if it doesn't come together right away
