How does this work? Is it a looped piece of tape that is constantly being written by the audio input and read by the car deck seconds later? Or does it do away with tape entirely?

It's a magnetic head housed inside a cassette shell that contacts the playback head, producing sound. No tape, and the "reels" spin only because the deck will always spin in playback mode.
