you mean the Spotify downloader right?
1. download the .rar and extract
2. double click start.bat in the folder
2. now it will open up cmd.exe and then it will automatically open your browser
3. you see now a local hosted website which asks for your spotify login data(just create a second spotify account if you are too scared)
4. go to https://play.spotify.com/ and search for the song/album/playlist you want to download
5. copy the link of the song/album/playlist and paste it into the bar where it says "Song or Playlist URL"
6. it will now start downloading everything from the link you pasted in, into the mp3 folder which is located inside the spotify downloaders folder

I checked the file with virustotal and not a single threat was detected. This program is written in javascript, so it should be EASILY detected by any antivirus if it was harming at all.
It is also featured on a big german tech-board since over 5 months. The people there would definetly know if it was suspicius.
You can also watch if the program creates any connections with a server(which it doesn't) which steals your Spotify login data if you wish. But that shouldn't be a problem at all, since you could reset your password.
But like I said, create a virtual machine and a second spotify account if you are so scared
