The good thing about American Psycho is the ending being ambigious. It's neither "it was all a dream" nor "look how nobody cares, modern society everyone".
The topic of the Patty Winters show stated in most chapters gives a good guidance. If I remember correctly some of the less fantastic murders correapond with realistic topics (stabbing the first hobo iirc.). The most gruesome acts are probably in his head.
