There aren't many in my list because I usually just buy them. Here's a few examples of my book buying sprees (Usually every week, sometimes a few times a week).I think I have a problem.

For every book I read, I add about 2-3 new ones to the wishlist.

I'd say the vast majority comes from other literature. When it comes to non-fiction the references/recommendations tend to be explicit, so if it seems interesting or if I want to learn more about some particular subject, I'll look it up and maybe add it to the list.
When it comes to fiction sometimes the references can be explicit (e.g. Pynchon going "Check out Ishmael Reed" in GR, or Borges going on about some author he likes), but usually they're oblique and I have to hunt them down. Another usual avenue is reading criticism or reviews of something I like: there tend to be comparisons to other works, or "this was inspired by/inspiration for X" which lead to new additions.
If I really love a book I just add the author's 3-4 most famous books to the list.
I don't add them indiscriminately, I tend to read the blurb and reviews first.
This is page 27, I added these around October 2010. This selection is a bit fiction-heavy, the list as a whole has more history/philosophy.
