The "woman trapped in a man's body" explanation of a transgender person's identity is referring to the 'man's body' being sex as defined by chromosomes while the "woman" being the abstract and socially dictated aspect of someone's identity, which is taken involuntarily by the person as being the more accurate concept of the correct state of the body. In OP's dichotomy all of the terms are taken as referring to the same thing when they aren't meant to be taken as such.

Gender in the way most people intuitively think about it is defined by social understanding. The basis of it is biological differences but they way we enact it is constrained by our conceptions of our social role.
Little harsh to call what you have a disorder though, it depends on how debilitating it is to your livelihood. Lots of biological males are much happier taking female hormones and living as a member of the opposite sex.

There's hardly evidence of a body map problem. The processes that differentiate the brain pre natally can cause a predisposition to becoming trans (just like it is for homosexuals) but there's no clear evidence the cause is a body map problem. It's something which can be observed and measured, but until then we can only speculate it. Really it seems unlikely that you can 'fix' faulty wiring by simply rearranging the preexisting tissues since it's still going to be the same nerve connections sending signals to the brain. It also doesn't explain why some people who identify as trans barely have any issues with their genitals and are focused on other aspects of gender.
