Get cheek fat removed (relatively inexpensive <1k), pluck & fill eyebrows (get the eyebrow kit by gwen), you're going to need more eyeliner than that, and maybe lip injections, not sure how rich you are. The cleft chin is actually something I see on girls and it would look cute if the rest was a little more delicate.

Surgery related stuff ( not recommending it but if you're going down that route):
1. get rid of butt chin
2. need a femenine nose
3. need higher cheekbones + make them less wide
4. Ears are not large but could use with a little shortening
Stuff you can do now: pluck your eyebrows in a more girly/flattering fashion
Lose weight: you have face fat.
