They are reliable when they've been checked and head spaced. The TLDR is that some parts get worn out at different rates and you might have different aged parts on a weapon. You also need to make sure that a previously headspaced weapon is back at that exact configuration when you put it back together after taking it apart to clean.

You use a small metal key that has the min and max widths on different sides. You make sure you can stick the min side of the key in, but can't stick the max side in.

Headspace is the distance between the face of the bolt and the base of the cartridge case, fully seated in the chamber. Timing is the adjustment of the gun so that firing takes place when the recoiling parts are in the correct position for firing.

The M2 is a large weapon and to ensure it functions properly you must use a metal device to ensure these things are correctly set.

Every headspace and timing gauge gets double checked with a micrometer at the factory and in QC, than again every year when in use.
The stainless steel they are made from is harder than any part of the gun.

A liner is a box ammunition metal m2a1, 100 rounds of .50 cal.
4B1T is short for four ball one trace link.

Last person to clean that gun didn't check the timing. Basically there is an adjusting wheel inside the receiver under the rear sight that controls the height of the trigger bar which control when the sear drops.
This happens when people go overboard with the cleaning and accidentally turn the wheel over and over again with a cleaning brush. Basically the trigger bar gets put in a position where it cannot reset the sear.
Or it could be the gun is clapped out. The Marine Corps literally has WWII vintage M2s with wood grips in some infantry battalions..

That is the obvious problem, but if you slow it down to quarter speed you can see he is feeding and ejecting when he performs immediate action 3 times in a row around the 2 minute mark.
Tripod mounted M2s have even more sever belt hangs and they fire fine.

Oddly, I've seen many many civvies using aircraft.50's on ground mounts, firing entire belts of poorly stored surplus ammo.

I'm sure there are vids from the Big Sandy MG shoots showing water cooled or aircraft. .50's shooting for prolonged periods with no issues.

I'd personally check headspace and cleaning discipline.
