broken case or some sort of case failure. could have been out of spec round that detonated outside of fully seated chamber and blew off the rim. it happens to all rifles. over charged round, weak points in the metal of the shell, lots of reasons. especially the dirtier the chamber gets. the case will get stuck and the extractor will rip the rim off or chip it, and boom, you have a round that cant be removed without sticking a rod down the barrel. you can also get a tool that goes into the broken shell and gives the extractor something to latch onto
