a carry handle has two holes in the top for mounting optics since the 90's. you can get picatiny rail adapters and everything else.

the only downside t this setup is increased vertical profile.

A "combat" rifle is zeroed for, generally, either 200 or 300 meters. The ACOG is zeroed for 100, with the bottom of the chevron being point of impact at 300M. Precision rifles are commonly zeroed for 100M, then dialed for exact distance, making point of impact the same as point of aim. The idea of the longer zero in combat rifles being that out to an okay distance you will hit +/- center of mass without much in the way of trajectory consideration. Point rifle receive effective hit, basically. Whereas the precision rifle is geared towards dialing for a specific point of impact.

Here are some example images of sight picture when engaging a person sized target at 300m with an ACOG equipped rifle. Basically you put the chevron on their chest in most common engagement scenarios and you will hit them somewhere in the chest or upper abdomen.

Yes, infantry soldiers usually are imparted with a general understanding of their weapons' external ballistics. I went through U.S. Army Infantry OSUT in 2010 and we were educated on engaging targets out to 300m with our Aimpoint equipped M4's. Further training was done at my unit on utilizing the ACOG, including range estimation using the reticle (handy when the opportunity presents itself, you need the target to be facing you so you can line up his shoulders with the corresponding width reticle subtension). For general use the ACOG is a great optic.
