The American Healthcare system doesn't really have a quality problem; it's one of the best in the world at delivering good care. It's above average in most areas.
What we have is a cost problem and an access problem with both of them interacting to make each other worse.
There are 101 reasons and more why the system is so costly with everything under the book that you can think of having a sizable to small negative effect; it all adds up to one big fucking mess. This also means that there's no one thing that will fix the system like a panacea.
