Artificial creations of Allies after breakout of Yugoslavia to bully Serbia:
Montenegro (they're just Highlander Serbs who didn't got annexed by Turks)
Kosovo (seriously, what we need is more muslim countries in Europe)
Macedonia (West Bulgaria)
Maybe even partition Bosnia too, between Serbia and Croatia. I'm not sure about their existence's legitimacy.
Balkanisation of Low Countries: (Holland+Belgium-Luxembourg) should be one. I still can't find more artificial creations than Belgium aside what happened to Yugoslavia in Europe. Just perfect capital for anti-national cosmopolitan EUnionists, are those alienated people.
Small-states, like Monaco, San Marino, etc. Just make them autonomous like Hong Kong or Macau. Not sure about Andorra.
Merge Vatican with Rome, make it as such same for former Constantinople. Those are holy places of Christianity, aside Antioch and Alexandria and capital of their respective denominations. Capital of Italy could be in Turin again.
