The problem is that even when a medical problem isn't caused by their smoking (or obesity for that matter), it still fucking complicates things. It's a joke how many times I had patients sent to the ICU I was working at earlier this year with nothing but a call from the anaesthetist saying "I have a patient on the table with a history of COPD (or sleep apnea) who's going to need monitoring post op". So they get admitted to ICU, which costs ~$3000-5000 a day, plus extra costs for ventilation and monitoring. It's fucking ridiculous.

The main danger in smoking comes from incomplete combustion products (carbon monoxide, tar etc.), self rolled cigarettes are actually worse because the higher moisture content in the tobacco means there's more incomplete combustion.
