And what would you do? Replace their governments by force? Set up huge organisations that would rival governments in their countries to distribute money probably against that country's government's will?
International aid could be more effective but making it effective would be coercion and inflammatory in and of itself too. Western countries do the best they can.

Those are all long term goals. I doubt the current policies can endure long enough to achieve that.
