I didn't know the answer straight away. We've done the opposite multiple times, but a state merger is apparently not unconstitutional, or inconceivable. My understanding is that it's like the mafia though - once you're in, you're in forever, and that's it.

New flags will be made with 51 stars, and they will slowly start to replace the 50 star flags.
Government buildings will likely change out their flags straight away, but it honestly wouldn't be a very large expense.

Well, the flag I posted with the OP is actually officially declared to be the one that they would use in the event of a new state by the government, and it changes the alignment of the stars, which makes it a bit more obvious.
I wonder if there are any places out in the middle of nowhere where there are still 48 and 49 star flags lingering about. I doubt anybody would ever notice.
