For the voiced th sound, put your tongue between your teeth and make a z sound. For the voiceless th, make an s sound instead.

When you're supposed to make one or another: just memorize with the word.

And IMHO this "H digraphs" convention only works if
a. your language has no sound you want to use H for, or
b. you're using the digraphs for aspiration.
