America is very big, as such it costs a lot of money to lay cable. Cable is expensive. Because of this there is only a few companies willing to do so. When there is only a few companies doing something there is no competition, so they couldn't possibly give less of a fuck about you or their product.

It's the thing I probably hear the most complaints about on a daily bases. Time Warner Cable at least has to compete with Verizon Fios where I live. Sadly a lot of the mid west is comcast territory and they suck the worst.
