While astronaut Gus Grissom was waiting in his capsule for the helicopter to pick him up, the hatch on the side of the capsule, for whatever reason, blew.
Instead of going for the astronaut first, the helicopter crew decided that the capsule was a higher priority. The capsule waterlogged and sank, and Grissom nearly drowned. There's still no consensus on whether that hatch blew on its own, or whether Grissom did it himself.
