You need more than two sentences for the plot of Netoraserare: A married couple who are also childhood friends desperately want a baby (moreso the wife), the husband feels too much pressure on him and resorts to revealing his secret NTR fetish to achieve their goal.

Sempai is actually relevant. His arc is the most important in the series. Everything that is happening now is directly because of his involvement. If you don't care about sex scenes, then you got something wrong with your head since Netoraserare is a hentai manga. It fails as a hentai manga, more so as a story. There is no character development during the series, Haruka is just trolling Shu. The Sempai Arc is the best part of the manga and should have ended there.

Also I wouldn't say loving through suffering since Shu wanted Haruka to stain herself more and more. He doesn't feel normal love for her anymore, he is obsessed with see if Haruka being NTRed and only cares about himself. Just save your judgement of the story being shit until it's over or at least the next chapter. If it's the same thing again like in chapter 21 and 22, and the time skip is pointless, I could agree with it being disappointing, but I personally don't think the Manga will go on with just an increase in the sex partners and Haruka getting pregnant after years of sex from some random guy.
Also, sluts and suffering through love / love through suffering have nothing to do with each other.

While Shu doesn't care about Haruka's feelings anymore. Shikishiro is clearly taking ideas from Osamu Dezai's No Longer Human. I won't be supposed if it ends like No Longer Human, Haruka's parents interfering (Because Kawai informed them) and leaving Shu single. Kawai and Shu then hook up and get married while Haruka suffers from PTSD and Senpai eventually rescues Haruka and takes care of her. Time skip after a year, Shu and Kawai are in the bed to have sex and...Shu's NTR illness shows up again and ask Kawai to sleep with other men again. Meanwhile, Haruka become pregnant and gave birth to twins. At the final page, it will show the joy in Haruka's face and shock in Kawai's face (unable to believe she isn't enough to Shu).
In the omake, Shu watches Kawai having sex with random men with the expressions he always have while Kawai is having sex with the man with a dead fish expression.

Shikishiro is really trying hard to write like Dezai and he is failing miserably. If anyone even hope to fully understand Netoraserare, I suggest reading Disqualified From Humanity aka No Longer Human, specifically Memorandum 3. Netoraserare will look like a bad fanfic of it.

Takakura-sempai was to be the last arc with an pilogue/timeskip where Kawaii was supossed to be important somehow.
One of these was the original ending:
- Haruka leaves with sempai, Shuu gets Kawaii and she gets pregnant.

She did have a choice: Actually having a proper conversation about him and his fetish. The entire story is based on a lack of communication between Shu and Haruka. Them taking therapy didn't happen because this is a Hentai Manga, though.

As to why he asked her in the first place: The pressure of getting a baby, both by their parents and Haruka herself overwhelmed him and kekolding was his only way to be able to perform himself.
Read the first chapters again. He was fine for quite a while after the first session and could perform himself, until he couldn't get an erection for some reason anymore and resorted to kekolding again, which got worse and worse, like a drug addiction.
