That guy died because the neck piece was too tight and it interfered with the blood flow to his head. Blood pressure changes will fuck you.
IIRC it was a head out vacbed that used a neck gasket, what that means is his head was actually outside and thus had a specially made seal around his neck to prevent the vacuum cinching it in too tightly. He wasn't using a bed/seal custom made for someone his size so it was too tight and thus didn't function properly.
