They work just fine. I used to have one until it broke a few months ago, now I'm using a normal mouse again and hating it.
I still got a sore wrist once in a while but not as often as I do now.

OP pic is still my favorite trackball but they've been out of production a few years now so a new one will run you $200+, the Sanwa and Elecom wired trackballs are the closest thing to it that you can get at a reasonable price now
Also don't throw it away if the buttons stop working after a few years, it costs about $5 for new microswitches and if you can work a soldering iron then you can replace them yourself
