Ok so I've had my current desktop for about let's say a little under 6 years... I built it to play Skyrim on Ultra Settings and it pretty much worked fine... The only time my computer crapped out was when my video card started failing and I had to get a different one... It's literally the only other part I upgraded within the 6 years.
Mobo : Asus Crosshair Formula 4
Video Card: HD Radeon 7870
Ram: 8 gigs of Ripjaw ram (don't remember which ddr it is)
Heat Sink: CoolMaster Hyper 212 (This thing is freaking enormous and I definitely need to change it to something better)
CPU: AMD Athlon Phenom II Black Edition 3.2 GHZ
Case: Cool Master 690 II
What do you guys think I need to replace?
I was thinking I could get a better case, motherboard, cpu, and heatsink... I could probably use a better video card but it's been working quite fine for me so far.
I'd rather stay with AMD if possible.
Add a GTX 480
It stays cool and has great performance!

Well, I don't see any reason to upgrade yet if you don't need to now. You should be fine until it really starts showing signs of slowdown.
Maybe get an SSD for booting if you don't have one already?

The only reason is because if I want to run a game on high settings now a days it's not as fast as I'd like... I mean let's say I want to run Fallout 4 on high settings.. This system wouldn't really cut it.
