GIMP's biggest problem is that it's not being made by people who really use it for anything, the most they'll do is scribble a bit with the paint tool or adjust the color balance on an image. Doing more major work begins to fall apart rapidly. As an free (gratis) and free (libre) project, the devs don't have an incentive to really listen to the users or even worry about competing projects/products.
Saying "users can go fix it themselves and submit a patch" just leads to a shitty piece of software, most users care quite a lot about the lack of a feature, but not even remotely enough to go add it.

Blender has a bit of this problem too, although it's a damn sight more competitive with the pro products it's in the same category as than GIMP, even if it's still not terribly great.

The scripts are a pain to install for normal, average users. Sure, I could go download the script, setup Perl on this machine, edit my GIMP startup to run the script at launch, etc, but for the regular Joe using GIMP? Probably not.

Is there a maintenance cost to all of this? Sure. But that's not even the rationale the developers give when they reject the feature requests, that would be a more reasonable, understandable response than anything I've seen.
