Don't use any Google services. Just install Cyanogen without gapps. Use F-Droid instead.

If you're running a ROM that doesn't include it in the settings (e.g. factory ROMs, pure unmodified AOSP), yes.
There might be an external app for it too, I don't know about that. I've always done it through a text editor since it's just one simple line anyway, and I use the factory 5.1.1 image on my Nexus 4.
