You have to maintain the lines (which cost money) and implement QoS (so that you can oversell the lines). You have to build hubs and datacenters (for caching so that you don't have to download files from the other side of the world through 30 hops, and to minimize line length) and you have to actually lay the lines as cities evolve.

Most sites big enough have servers everywhere. Smaller sites can pay their hosting company to have their sites expanded with more hosts. It's all paid for via advertising. What am I missing?
