I use my brain for system password and Blur (Firefox) for everything in the web except some personal things.
keepass, but 99% of everything i use are all sharing the same 6 or so passwords that vary in strength

If I can login with Google or Facebook I always choose to do so. My passwords for those sites are complex and require two-step verification.
All others I just let Lastpass generate passwords for me.
