If free healthcare is implemented, people with bigger problems get ahead of the line.
This means that you might end up waiting multiple years to get rid of your ridiculous gyno or maybe create an actual gyno so you can wear a dress (since both of those are aesthetical and not that important so they can wait).
But this also means that if you have a broken leg, it'll get handled ASAP. Which is why the other people are waiting so long to begin with.
Of course, there will be people abusing this system and there will be bad doctors corrupting it from the inside. But it can work relatively well. It'll just be uncomfortable.
