I blame mostly his mother for not letting him go so he could actually have a life and his fucking sister's (now fiance) for getting him hooked on that shit in the first place and probably doing it with him the night he died despite knowing he was just in fucking rehab.
Obviously he made his own mistakes and whatnot but she is not free of blame, either. She just had to drag him into her shit, passing her mistakes off onto his shoulders.
Here's his last diary entry made the day before he died:
