There's conflicting studies on whether or not the aspartame used to sweeten it causes insulin spikes or cancer. It's definitely not good for your teeth. Probably not going to harm you much if you drink a can or two a week.
I'd just stick with water.

Don't consume it with a meal, it causes acute changes in several metabolic neuropeptides (it's not gonna affect insulin resistance or blood sugar, it's just gonna slightly increase the activity of orexigenic peptides like ghrelin and NPY). It will decrease satiety of a meal, but it's not necessarily bad.
It's not gonna give you cancer or diabetes.
As fas as for weight gain it doesn't affect it in one direction or the other what I've seen from studies. As for health effects I haven't seen any credible studies that would prove it is harmful for people.
A waste of money it is.

I think the worry is that there's very little long term studies on sugar substitutes because they aren't as old as, say, tobacco, which we always knew was bad but never quite knew how bad until a huge amount of old people got lung cancer.
I usually have a rum and Coke Zero or Diet Coke because I'm on a permacut. I don't really drink it otherwise but I don't think it's ever been proven to be particularly bad, credibly, but Id understand some apprehension about it.
