Go in one day, guy squatting, blood shoots out all over from his nose, looses balance, plates slide off lands on spotters foot, barbell flips over knocks other spotter down and gets embedded into the concrete block wall, this cause several other drops and stumbles from others, and ends with the squat rack crashing through the window front of the business.

Same thing sort of happened to me a few years ago at my uni gym, but I didn't miss the rep.

I was reracking the weight after doing squats. I guess I didn't place it all the way on the pins because as I was ducking under it to leave the right side fell off the pin and the bar hit me in the head. I'm pretty sure the only reasons I'm still alive is because it was only 185 and it hit me before it had enough time to accelerate. I checked my head by running my fingers through my hair and I couldn't find any blood. Now I always do a double check after racking the weight. Be careful guys... you can die doing squats, it's not just bench.
