How to I do heavy barbell shrugs without mashing my dick every rep? I can't justify spending $$$ on a trap bar.

On a related note, anybody else get extreme testicular torsion when doing back extensions? My balls practically end up in my stomach by the end of a weighted set. Can't do deadlifts for a while due to knee surgery so trying to compensate.
