you order one online, or if you have a local screenprint shop you can maybe pay them to do it.
Or you can get into screenprinting in which case I would recommend taking a class because most people who try it on their own fail miserably and give up.

I screen print my own tee's and it's pretty easy if you don't do too many colours, because creating multiple colours requires more than one screen and so you have to make multiple screens and seperate them according to colours and then line them up perfectly its a bitch. For a b/w print though just get wood, nail it in a square, stretch silk screen material or thin curtain material if you're a poorfag, buy some emulsion and ink (you can get it in a pack sometimes) and then watch a couple youtube videos on how to do it easy peasy. You're gonna fuck up alot the first time though

There's this special t shirt transfer paper you can buy. You print your image and then iron it onto the fabric. Getting the colors right can be tricky but for the most part it shouldn't be a problem if you're going for that particular design. The I lay real potential issue is longevity
