In the real world? no one is sure if a non-sterile hermaphrodite human is possible, there are no cases of both organs being fully functional. There are no known existing hermaphrodites with both organs fully formed either. The troubles associated with inbreeding are another issue, impregnating yourself logically should be the same as or worse than impregnating your twin opposite sex sibling.

There's basically no way for a futanari to exist naturally in real life.
