I always thought it had something to do with (what appears to be) those dinosaur-like scales along his back. But now I'm assuming it's like, a gay/trans thing?

That's just the background character's hair. See, there was a time when the Simpson's animators/creator/director/storyboard artists actually cared enough about the detail of the shots they were producing that they would make sure everything would logically make sense when viewed from a certain angle. It's easy to see your confusion, given the current state of the show. Hope this helps.
