There was never any indication before that point that Carla was transsexual, much less any indication that Mary knew about it. Physical clues were hampered by Willis' artistic skills, implications were more or less passed off as elements of Carla's impulsive and loud-mouthed personality, and unless there was some Patreon strip that's never been leaked until now, Mary and Carla never once had any interaction before the scene where Carla was skating up and down the halls.
