The manga Family Compo by Tsukasa Hojo (City Hunter, Cat's Eye) did it. Basic premise is a guy goes to live with his aunt & uncle after the death of his father (his mother had died years earlier) and discovers that they're both trans, which leads to a sense of crisis as to the identity of his cute but rather gender-fluid cousin. I don't know if it was ever ultimately resolved, I never finished reading it (slow scanlations years ago, fell by the wayside).
And, you know, for a manga written twenty years ago, it's remarkably adept at handling trans issues.

I wish they'd just kept it how they did in the first few episodes, with her genitals just being a bulging obscure mess and never explicitly referring to her having a dick
