Alan Moore is notorious for ignoring continuity and just doing his own thing with the characters. Jack Kirby did this as well. There are some writers that can do a good job of using continuity to tell their stories (particularly Morrison and Busiek). But for the most part, adherence to continuity has no relation to the quality of work. Ewing loves his continuity but he's overall a pretty mediocre writer. Bendis is shit when it comes to continuity, but that's not what makes him a shit writer.

I wouldn't count Fantagraphics as mainstream, but they've been consistently great for a long time. I'd say Crossed is the only mainstream book from Avatar. Patience and Providence are indeed amazing, but I don't consider them mainstream and personally I prefer Prophet to Providence. There's still a lot of great comics coming out (Beautiful Darkness, Andre the Giant, My Friend Dahmer, those are just a few off the top of my head) but there's nothing really great coming out of the Big 2 like there was in the 90s or early 2000s. I like Deadly Class too but Remender's whole tortured childhood schtick is getting kind of old.
