Toph's speed < Azula's speed < Rock Lee's speed < Gaara's sand
How is it that Toph could hope to fight against gaara when her body couldn't hope to react fast enough to the sand?

it takes, like, 1 second for Toph to smash his skull. 1 second is what takes for him to realize what's going on. Even if substitution is indeed instant, he still have to prepare it, decide to do it, etc.

the entire technique took about half a minute, from beginning to end. At that time Gaara was already waiting for the window of opportunity to use the technique. He was already expecting to use it, and he did in the middle of the lotus. Again, it is instant, but you still need to decide to do it. Gaara wouldn't even be able to know what is going on before she smash his skull. Not only that, even if he did get away, she would know where he is.

it all comes down to speed. Even though Gaara may be physically one of the slowest ninjas, he is still a ninja making him faster than normal humans...which, though blind, Toph still is.

Gaara is more agile, but how much is debatable. We know more or less the speed of his sand (highly overrated by fans) but that's about it. I wouldn't say speed in naruto is inconsistent, but fans tend to get anything that can slightly be used in their favor. I remember that one thread with 3-4 narutofags defending that every character in the show is faster than light, and that X and Y technique is faster than light when in the manga itself there is no such statement.
Sneaking on the sand shouldn't be an issue considering Gaara can't "feel it" and she wouldn't be crushed by it.
the real issue is how can they tell what Toph can and cannot do with sand that is being controlled by other people. That's very subjective and i think death battle weighted too much her bending because "there is also metal in Gaara's sand armor".

From watching that episode, I'd say "seeing" through sand is like us trying to see in a sandstorm. She could make out blunt objects but she isn't going to be that precise.
Gaara can fly and Earthbending in general is generally made of straight and direct shots.
Toph wins the death battle because for no reason, Gaara stands completely still in the air for Toph to rise up 200 feet toward him because she can only rise directly upwards.
