Big Head, the one from the actual mask comic, was murder maniac crazy. Anybody that got the mask would go homicidal maniac in no time at all.

Thom Kallor, the Legion's Starman was pretty screwed up mentally from falling through the universe. He actually is institutionalized in the "present" DCU and described as schizophrenic, but he's pretty harmless apart from just talking in vague fourth-wall breaking foreshadowing.
Speaking of Starman, Ted Knight was written by Robinson in the Golden Age Elseworlds as going through a severe period of depression following his involvement in the Manhattan Project, and Johns retconned that into his JSA run later on.
Thom Kallor is schizophrenic. Moon Knight has DID.

There's Spiderman, Batman, Dick Grayson with survivor's guilt complexes, The Punisher Frank Castle with his psychotic rage, Electra Nachos with sociopathic tendencies regarding killing, half the cast of X-Men comics need a padded cell, Scarlett Witch, Steven Strange, Dr. Voodoo have all three lost touch with normal reality, Captain America (Rogers) had a Bucky guilt complex for half a century, Tony Stark is a dangerous narciscist beyond compare, Bruce Banner/The Hulk is a whole bus-load of crazy,
etc. etc. etc.
