Animal Man v1 has got to be a solid contender. Brian Bolland was easily one of the greatest cover artists of all time.
Tim Bradstreet's Punisher covers were really striking despite being samey.

I'm partial to Mignola covers myself.
Though technically brilliant, they're starting to look the same however
I'm not sure about best ever, but the covers of the most recent Batwoman series were absolutely gorgeous.
NOW's X-Men Legacy had a ton of fucking great ones

Uncanny X-Force had pretty stylish, yet informative covers.
The Unwritten series has the absolute best covers.
The only comic series I purchase in floppy form, just to have all the covers (I usually buy trades).
The story was aces as well.
