They get off to one of two things, other peoples comments on their girls, or seeing other people repost their girls. Now, while that is totally doable on /b/ alone, picswap facilitates the instant gratification of both someone rlse rating your pic (by saving it), and seeing someone else posting it. That is why picswap is worth something, and why simply posting on /b/ gets pretty boring.
