The first time I attended the medical screening for Gulag, I had to fill out a medical questionnaire.
There was a section on social history: "Have you ever had tattoos?", "Have you ever been in jail?", "Have you ever done drugs?", etc.
"Have you attempted suicide?"
I ticked no.
In the "misc comments" section for that question, I wrote "Not yet."
As far as written records go, I do keep a log of my most personal thoughts, and I've been keeping it for two years. Safe to say it is well-kept on a fully-encrypted SSD with a 16-character password. Nobody can access it without my authorisation.
