If you download the file from the "the last user uploaded..." screen it's always a "pictureswap" filename. If you rightclick > show image the image, it shows you the original filename.

But to be precise, it doesn't work the way it's supposed to. It doesn't give you the image the "last poster" uploaded, it gives you a random image that was uploaded during a specific period of time. That's why you can F5 your way through the "last image..." results.
