I think I understand it. The child in the two photos are the same person. It is picking on the "be careful who you call ugly in school meme". The creator of this post probably has severe depression and this is how he deals with it. He channels his depression through comedy. When in reality, killing himself at Sonic Drive Thru doesn't sound too bad to him. By channeling it through comedy he can both take some of the gnawing depression away and help others who suffer the same problems laugh a little. His grandmother brings life to the post by showing how innocent a child really is when she comments about the poster's baby photo. The poster then replying with a cliche cringe response helps the final punchline of the joke. Finishing it up with his grandmother apologizing after realizing she made a very cringy moment.
