I'm sure culture plays a part into it, but any rational hockey league manager, ref and committee would see that only trouble comes out of it (players getting disabled from playing, etc.) and would try and do something about the current rule-set

Fighting is just part of the game. Most times (unless a player actually done some fucked up shit) they aren't actually that mad at each other. They stop fighting voluntarily after they can tell it's over, and are often seen telling each other good fight.

In the early nineties the instigator rule was introduced and destroyed the chemistry of everything by trying to make it more palatable to the average sports fan. I don't know if this has been stated or not, and I'm not entirely sure how genuine your question is....BUT

There is a role within hockey titled "bruiser/goon/enforcer/etc" that essentially bullies opposing teams and also protects their own teams stars.

The point is to rattle the cage of the other team by delivering a lot of hits (scott stevens is a prime example)

Also, typically teams have one of these types of players on a certain line to either go head to head with the opposing teams brute or to bully around a high skilled player.

Most fights are often agreed upon between the two players at a face off or in the chat of the "mob" like jams when a bunch of players group together.

This can achieve a crowd or moral swing for a team. or to go back to the protection aspect.. if you are a bruiser, and your star player gets wrecked, you're the player to go fight whoever did the damage.

It's a vital role in hockey, and yes is decently contrived, but it is typically for a purpose.

That's not to say some fights aren't just bad blood or a frustration from the course of the game.. because certainly this happens.

However, i think it's a dying role within the NHL and needs to be reinstated with more relevance. I miss the Scott Stevens, Adam Foote, Zdeno Chara, Rob Blake, Chris Nilan, Tie Domi, Donald Brashear, etc..

I don't necessarily believe it's a fear. I think it's more due to the development of the game. Goalie pads are bigger than ever, and also goaltenders themselves are more and more technical and athletic...making a skilled offense more and more important. IE.. less enforcing, and more dangles. The speed and style of the NHL has become so much faster and precise, that goons are becoming less of a commodity.. Granted, I don't think this is a good thing, but it is more of a "evolution" of the game than a fear. IMO anyway.

Any headshots/boardings (or stepping out of the penalty box before the penalty is over, stepping on the ice for a fight or trying to break a fight) are 10 minutes game misconduct with an ejection and automatic suspension before the hearing with the player's safety department.
