Why do some wrestlers wear mouth pieces and others don't?
Is the mouth piece just part of their attire or is does it serve a purpose to protect their mouth even though punches usually never connect?

general level of importance: aesthetics > comfort (jaw tension, teeth clenching) > protection
Same reason some wrestlers wear MMA gloves, it's a little of both. It mostly looks the part but will also serve to protect your mouth from a stiff kick or a misfire. Notice how most people who use them wrestled in Japan, where they wrestle stiffer.

Probably because some wrestlers are stupid.
I would wear a mouth piece all the goddamn time. Less to protect my teeth and more to protect my brain. Having a mouthpiece allows something for the jaw and neck muscles to clinch against, and lowers the incidence of brain injury.
I'm 99% certain I'd have at least one more concussion and a broken jaw if I didn't have a mouthpiece for rugby.
Plus, certain guys look more menacing with a mouthpiece. Kurt Angle, for example.
