I've always liked SHAFT BD changes. I've always gotten the idea that they made so many changes as a kind of thanks to the people who actually spend money on buying the BDs. Not only do you get improved artwork, but you get some neat changes to scenes.
That, and I'm sure they have threads like this on 2ch as well, which basically act as free commercials for their BD releases.

Well, the director is usually in charge of that sort of thing. I guess there could be a different process, but I don't see a change like this being allowed without at least approval from the director. They're not arbitrary changes. It's just hard to figure out the motivation behind them a lot of the time.
