He's still technically Zangetsu. Zangetsu is the name of Ichigo's Zanpakuto, which in turn is shaped by his inner spirit. Ichigo has two inner spirits, just like the other Shinigami who have two Zanpakuto. One is the manifestation of his Shinigami powers (Shiro, the Hollow) and the other is the manifestation of his Quincy powers (Young Yhwach, Old Man Zangetsu). Together they make both blades of Zangetsu.
Before, the Quincy spirit forced the Shinigami spirit out to protect Ichigo. His Hollow transformations was the Old Man Zangetsu allowing Shiro to intervene, which is why they only happened when Ichigo was completely fucked.
As for plot holes, it didn't really create any because Kubo is the kind of writer that just scatters plot points through his work with the intention of maybe picking them up later if he remembers. There is foreshadowing, but it's pretty impossible to pick up on without this new information. It just turned everything we had established about Ichigo's powers on it's head.

When she took one of the bars and started scanning it multiple times, he stopped her and told her to scan them each individually “to prevent any reiatsu infetterence,” and then turned around and winked at me. I don’t even think that’s a word. After she scanned each bar and put them in a bag and started to say the price, he kept interrupting her by yawning really loudly.

According to Ichigo, Zangetsu never lied, so when he said "I'm Tensa Zangetsu" in the Dangai training, it was probably true.
But all in all, both the old man and the hollow always said they where the same thing, and how they appeared only depended on who was more powerful in that particular moment. Which, if you consider that basically his Quincy powers have ALWAYS been suppressing his hollow, means that he never was at full power. Also, there's a few things of interest:
Until recently, Ichigo only had a proper zanpakuto when he was using Rukia's.
Ichigo's OP reiatsu only really awakens when Ishida tries to use his reiatsu as quincy power to shoot his arrows.
Old Man has been able to create sham zanpakutos all the time, he does that both in the inner world, and when materialize by Ichigo for bankai training.
Ichigo couldn't actually hear the name the Old Man said the first time they meet.
In the end, Ichigo acknowledges that Zangetsu, ultimately, is just himself.

Fullbrings are abilities awakened by residual Hollow reiatsu passed from mother to child in birth. So if the mother was attacked by a Hollow before the kid was born then they become a Fullbringer. From the descriptions of the Fullbring ability it's some weird fusion of Quincy/Shinigami powers.

If you're talking about the scene where Renji is on the ground and sees the monkey with the snake tail that said it was his sword, that was probably a vision that his sword spirit manifested for him to see (and only him).
I don't remember Unohana manifesting her sword spirit, unless you're talking about when she flies around on a giant ray looking thing, but I'd be inclined to called that her shikai rather than a manifestation.
You are right that it wasn't ever said (to my knowledge) that someone's sword spirit could be away from their owners. I guess I just assumed it.

Not only that, but he also hinted that the hollow was the true Zangetsu during episode in the shattered shaft, because when Ichigo was told to pull the sword out of the box in his inner world he wasn't able to in the manga. The inner world collapsed and he turned into hollow. Only then was he capable of reclaiming his powers.

We don't know.
One theory is that Mugetsu was essentially a Quincy Letzt Stil, perhaps with some Hollow and/or Shinigami mixed in - just like Letzt Stil did with Ishida, it gave him a form with one wing-like part, granted him immense power, but caused him to lose all of his powers afterwards.
It's possible he might get a new, Vollstandig-esque Mugetsu form that is sustainable, just like Vollstandig is a sustainable version of Letzt Stil.

It's not entirely clear. With his powers all unified now he very well might be at the transcendental level of power he was at post-Dangai training. It's also possible he has that potential again but is still working up to it.
It wouldn't surprise me if Ichigo's final forms were essentially the ones he fought Aizen with, only a bit different since he can now consciously control all of his powers. Meaning his fight with Aizen was essentially Bachgetsu letting him skip ahead in his evolution, but at a great cost.

My theory is that it begun as a Fullbring meaning that Orihime's extreme emotional attachment to the hairpin gave it life, but from there on it behaved like an Asauchi and thus developed a shinigami-like spirit and powerset.
This is of course based on the assumption that a construct such as what Nimaya designed can be created addidentally/naturally, which is not impossible considering that some of the royal guard members are probably older than the creation of the Asauchi and yet they still had zanpakutoes.
Maybe the first Zanpakutoes were born as Fullbringed swords for Shinigami who became closely emotionally attached to their swords.

All ohf his plans hinge on stuff like that, most of the time he just sets up scenarios that CAN have good outcomes and works with what he gets. The original goal was already to make Arrancars when he made the Vizards and when he made Metastica (the hollow that killed Kaien).
