The problem with the toothbrush scene is that it put very heavy emphasis on description and internal monologue within the LN, and SHAFT can't adapt visual descriptions and cut most of the internal monologue in favor of a bunch of erotic moaning and a couple bits of dialogue/monologue. For the most part, Nisio's writing.
Koyomi Araragi falls in love with a vamp.
