Most of your actions are instincts and muscle memories.

You just fight them. Fights aren't planned out in real life, but are mostly based on reacting to your opponent. At most, the mind reader would only be able to predict moves a fraction of a second before they happen, and when you add reaction time, it would be of no help at all.
If it's a woman, as you've indicated, I'd simply beat her up. She would be too weak/slow to put up a fight against a man, and trying to analyze your opponents moves a split second before they happen would be more of a hindrance than a help.

So far, I think that the only legit ways to beat a mind reader is by teamwork or gang attack, as stated in >>137452678. One person with multiple thoughts isn't going to work because you'll always have a higher focus towards the true thought of action. But having two people (presuming that they can't mind-read each other) will work by their own different actions and team-p against the mind-reader.
